Your medical care team will consist of an attending physician and any of these providers: residents, interns, physician assistants and nurse practitioners. Your attending physician oversees all of your care and monitors your condition while coordinating the recommendations of any other specialists who may be seeing you. Hospitalists are attending physicians who work full time for the hospital, providing care for patients admitted to the medicine department.
Upon discharge, your hospitalist will contact your primary care physician to discuss additional treatments or testing, help arrange any follow-up care and prescribe medications. Residents and interns are physicians who have graduated from medical school. Nurse practitioners (NPs) and physician assistants (PAs) are medical providers that have completed advanced degrees and training in areas of specialty, such as hospital medicine or surgery. They work collaboratively with the attending physician that is caring for you. You will be visited daily by your medical care team who will listen to your concerns and answer any questions related to your care and transition following hospitalization.
Nurses and members of the nursing care team are stationed throughout the hospital. They promote patient recovery in a safe, competent and timely manner.
Registered dietitians work closely with patients and staff. Development and implementation of nutrition care includes education of the patient and evaluation of the effectiveness of treatment. Proper diet is viewed as an essential part of the healing process.
Physical therapists work directly with patients to help restore physical function, lessen pain, strengthen muscles and encourage independence in activities of daily living. Respiratory therapists help patients recover through the use of equipment that facilitates breathing and analyzes lung function.
The case managers will work with you on your discharge plan. The discharge planning process includes determining what services will be needed and arranging for those services so the transition will be smooth and safe.
